2009-2010 Online Registration for Confirmation Youth Ministry

You will be asked general information to help us plan for the confirmation year, including if you will attend confirmation on Sunday or Wednesday. You can change later, but we need an idea of how many students to plan on for each evening.

In addition to this online registration form we will need you to submit the following:

1. Youth Medical Information and Release Form/Covenant Form
2. Copy of Insurance Card
3. Parent/Guardian Registration Forms
4. $75 Confirmation Fee
